He was predeceased by: his parents, Jim Polasek and Rosie Polasek (Myska); his wife Marie Forrest Polasek; his son James Polasek; his brothers, Raymond and Roy Polasek; his half-sisters, Frances Barnes and Lillie Mae Schafer; and his siblings-in-law, Robert Foster (Bob), Elmira Rogers and Jannette Forrest.
He is survived by: his son Michael Polasek (Monica); his daughter-in-law Susan Polasek; his siblings, R. D. Polasek (Minnie) and Patty Foster; his siblings-in-law, Kathleen Polasek and Melvin Forrest Jr.; his grandchildren, Ashley Martinez (Paul) and Eric Carver (Melissa); and his great grandchildren, Kiias, Korbin, Karsyn Martinez, Aubrianna and Isabella Carver. He is also survived by numerous nieces, nephews, great-nieces, and great-nephews.
