He was predeceased by: his parents, James Cook and Della Cook (McCarrell); his former wife Mary Alice Cook; his wife Virginia Cook (Vasbinder); his son Steven Cook; his grandsons, Jonathan Sanchez and Bryan Sanchez; and his brothers, Albert Cook, Floyd Cook, William Cook, Robert Cook and James Cook.
He is survived by: his daughters, Tracy Bellamy (Joseph), Susan Sanchez, Linda De Salme and Sandra Blount (Mike); his grandchildren, Christopher Cook, Jerrad Cook, Curtis Cook, Coley Bellamy, Maryalice Bellamy and Mike Blount, Jr.; his great grandchildren, Alana Bellamy, Avery Bellamy, Eleanor Cook, Emerald Cook, Everleigh Cook, Madison Blount and Mackenzie Blount; and his siblings, Marlin Cook of Florida, Judith Rippon and Virginia Fortier. He is also survived by numerous nieces and nephews.
