He was predeceased by: his parents, Louis F. Urbanovsky and Lenora Novosad Urbanovsky; his former wife Patsy; his son Bruce Urbanosky; and his stepson Rex Smith.
He is survived by: his wife JoAnn Costa Smith of Plantersville; his sons, Jerome Urbanosky, Jr. (Jane) of Plantersville and Mark Urbanosky (Terry) of Ardmore; his stepson Rob Smith of LasVegas, NV; and his brothers, James A. Urbanosky of Caldwell and Thomas F. Urbanosky of Caldwell. He is also survived by 16 grandchildren, 16 great grandchildren along with numerous nieces, nephews, other relatives and a great number of friends.
