He was predeceased by: his parents, Henry Cook and Evelyn Daisy Lenzi Cook. He is survived by: his wife Nancy Beth Cook; his stepson Von Setzler (Sharon) of Caddo Mills, TX; his stepgranddaughter Shay Myers (Storm); and his stepgreat-granddaughter Jaycie Emery.
