He was predeceased by: his parents, J. C. Perkins and Thelma Chester; his sons, Jack Hayden Perkins Jr. and Patrick Lee Perkins; and his stepdaughter Kelly Ann Gaines.
He is survived by: his daughter Cindy Butler of Pickton; his stepson Shannon Gaines; his grandchildren, Hayley Gaines, Brittney Bradley, Hayden Hollen, Jesse Butler and Skylar Gaines; and his siblings, Linda Hall of Mount Pleasant and Billy Perkins of Sulphur Springs. He is also survived by several nieces and nephews.
