He was predeceased by: his wife Peggy. He is survived by: his children, Dick Dvorak (Marilyn), David Dvorak (Debbie), Debbie Morris (Don), Diana Peterson (Pete), Doug Dvorak (Sally) and Dana Dvorak; and his grandchildren, Jody, Stephanie, Tara, Kimberly, Ashley, Tiffany and Jamie. He is also survived by 8 great grandchildren.
