He is survived by: his sisters, Patty Frantom and Gerrie Jones; his sons, Keith Kouba (Ronda), Kyle Kouba (Judy) and Kent Kouba (Kim); and his grandchildren, Chris Kouba (Hillary), Justin Kouba, Kelli Wilkins (Chad), Jami Perry (Keith), Kara Disney, Katrina Abbott (Greg), Jeremy Burrow, Brittney Davis, Candice Thornhill (Evan), Kristina Trevino (Sevin), Brittany Reed (Jared) and Corey Reel (Lesley). He is also survived by His children, grandchildren and great grandchildren.
