He was predeceased by: his parents, Lullar Cook and Matthew Cook; his children, Sharon Cook and John Frank Cook, Jr.; his siblings, Henry Thomas Cook, Willie Jay "Bill" Cook, Roy Wann Cook, Jimmy Wayne "Jim" Cook, Jackie Franklin "Cookie" Cook, Mattie Jane Cook, Georgia Louise Cook McCain and Betty Sue Cook Shipley; and his brother-in-law Bill Jennik.
He is survived by: his daughter Sue Cook Chandler (Brad) of Wheeler; his sisters, Mary Ann Cook Jennik of Sand Springs and Evelyn Jean Cook Anderson (Chester) of Okah, Oklahoma; his grandchildren, Jana Chandler Bartholomew of Amarillo, Bridget Chandler Weatherly (Shea) of Wheeler, Sara Silva (Jeremy) of Justin, Matthew Cook of Amarillo, Joshua Cook of Lakehills and Jason Velasquez Sr (Charlene) of Pampa; his great grandchildren, Janessa Bartholomew of Amarillo, Audra Weatherly of Lynchburg, VA, Austin Bartholomew of Amarillo, Hagen Weatherly of Wheeler, Braxton Maul of Temple, Jason Velasquez Jr and Julian Velasquez of Pampa; and his son-in-law Allen Whitson of Amarillo.
In honor of John's memory, and to continue with one of John's life-long commitments; it is the family's desire that memorial contributions be sent to the Shriners Hospitals for Children, 2900 Rocky Point Drive, Tampa, Florida 33607.
