He is survived by: his wife Shirley Ward Forrest; his daughters, Tausha Elliott (Chad) and Molly Purvis (Royce); his sister Joni; his grandchildren, Brittany Petri, Rachel Westwood and Hailee Elliott; and his nephews, Michael Johnson and Don Johnson. He is also survived by five great grandchildren.
