He was predeceased by: his parents, Clareese Cook and Joe Bailey Cook; his brothers, Louis Cook (Bud) and Raymond Cook; and his brother-in-law Jay Lark.

He is survived by: his former wife Sherry Ann Sweeney (Sherry Baldwin); his daughter Heather Perkins (Tracy) of Robinson, TX; his granddaughters, Summer Wright (Matt) and Liz Perkins of Robinson, TX; his sisters, Peggy Segotta (Jim) of Raton, NM, Joann Bass (Dale) of Albequerque, NM, Sue Lark of Goodwill, OK and Debbie Ulibarri of Raton, NM; and his sister-in-law Florence Cook of Barstow, CA. He is also survived by numerous nieces and nephews.
