He was predeceased by: his grandfather Lance Cook; and his great-aunt Regina Cook. He is survived by: his parents, Reginald Paul and Christina Dawn Cook (Offill); his wife Reagan Cook; his sons, Gavin and Zane Cook; his brother Gage Cook; his grandparents, Lisa Cook, Carol St, Cyr, Garry Thompson, Debbie Elguezabal (Adolfo), Ralph Smetana, Vickie Cooke, Rick and Linda Offill; his great-grandparents, Roy and Jody Smith; his parents-in-law, Butch and Kelly Jessup; his sisters-in-law, Taylor Jessup (Joe Davis) and Blayke (Taner Shott); and his aunts and uncles, Shanna Smetana, Melissa Clements (Cody), Brandon Smetana, Liz Carrasco and Robbie Simpson (Jeremy). He is also survived by two nieces, four nephews; and his 157 brothers.
