He is survived by: his fiancée Debbie; his children, Michael, Tommy (Addison) and Shelby; his wife Terri Hensley; his sons, James David (Rebecca), Mark (Kesha) and Chad (Sumer); his grandchildren, Aria, Stella, Lucy, Fletcher, Cadence, Emery, Easton, Van, Raine and Aaron; his siblings, Corey (Priscilla), Barry (Laura), Wade (Rebecca), Ellen Dingmore (Norm), Summer Fitzgibbon (Michael) and Sarah Casey (Carl); and his aunt Ann McClure Card (David). He is also survived by nieces, nephews, and cousins.
