He was predeceased by: his parents, Jack Virgil Ballard Sr. of Birmingham and Jean Lind Ballard of Birmingham. He is survived by: his wife Patricia Ann Ballard; his children, Jodi Lind Smith of Mexia and Joshua Ballard of Ft. Worth; his grandchildren, Taylor Commander, Mackenzie Commander, Lydia Ballard and Jackson Smith; his great grandchild Kassius Chaney; his siblings, Charles Ballard and Jayne Ballard McDaniel of Birmingham; his children-in-law, Dustin Smith and Wendi Ballard; and his brother-in-law William McDaniel. He is also survived by nieces, nephews, and many close friends.
