He is survived by: his parents, Jose Alvarez and Martina Alvarez; his children, Rey, Ava and Zachary; his wife Crystal Alvarez; and his siblings, Lourdes Cook, Joe Alvarez, Carmen Valetti, Jose Alvarez, Jesse Alvarez and Minnie Greer.

In lieu of flowers, the family would like memorial donations to be made for Ava and Zachary's college fund.
