He was predeceased by: his parents, Antonio Perez and Eloisa Perez (Acevedo); and his brothers, Antonio Perez, Jr. and Juan Perez. He is survived by: his wife Irma Jarmillo; his children, Noelia Hernandez (Juan), Esperanza Kovar (Dave), Eloisa Hernandez (Lucas), Eloy "Tony" Perez (Janet), Alicia Perez, Elizabeth Perez, Jonathon Perez and Abraham Perez (Andrea); and his siblings, Elizabeth Liscano, Ofelia Castillo (Manuel), Sylvia Castro (Rafael), Elvia Perez (Ricardo), Ricardo Perez, Samuel Perez, David Perez and Joe Perez (Rachael).

The family requests that in lieu of flowers, memorial contributions be given to the benevolence of the family.
