He was predeceased by: his daughter Samantha Gabriela Ontiveros; his brother Roberto Ontiveros; and his parents, Martha Sr and Candelario Ontiveros Sr.. He is survived by: his wife Veronica Lyzette Flores (Ronnie); his children, David Xavier Ontiveros, Michael Alejandro Ontiveros, Armani Flores, Candelario Eli Ontiveros, Ryan Samuel Ontiveros and Mariana Ontiveros; and his siblings, Melecio Ontiveros, Antonia Ontiveros, Jose Ontiveros (Nancy) and Cesar Ontiveros. He is also survived by nieces and nephews.

In lieu of flowers, the family asks that donations be directed to the Vannie E. Cook Jr.
