He is survived by: his wife Gracina Cook; his children, Cayla Floyd (Coursey) and Carl Cook Jr. (Elizabeth); his siblings, Vicki Cook, Terri Cummings, Jimmy Cook, Todd Cummings, Eugene Cummings, Derek Cummings and Tyrone Cummings; and his grandchildren, Oliver and Olivia Cook.
