He is survived by: his wife Wendy Perez; his children, Kimberly Perez, Audrey Perez, Virginia Ann (Eric) and Damon Joshua De La; his mother Minerva Alvarez; his brother Elbert; his children, Michelle Puente, Michael (Dorothy Perez) and Natasha (Ralph Placencia); his sister Sylvia (Rick Sandoval); his niece Natalie; his brothers, Adam Perez and Paul (Melissa Perez); his nieces, Katy (Rucker Olive) and Emily (Patrick Chandonnet); and his great-nephew Roland.
In lieu of flowers, the family has requested memorial be made to Mental Health America.
