He was predeceased by: his wife Jerrie Cook; and his son Jim Cook of Portland, Tennessee.. He is survived by: his sons, Philip Cook (Kristen) of Richardson and Chuck Cook (Michelle) of Flower Mound; his daughter-in-law Lyn Young Cook of Portland, Tennessee; and his siblings, Larry Cook, Cynthia Patrick and Judy Roark. He is also survived by his ten grandchildren, and nine great grandchildren.
