He was predeceased by: his father Dennis Belknap. He is survived by: his wife Camille Belknap; his sons, Jacob Belknap of Lake Isabella and Joshua Belknap of Lewisville; his mother Maurine Belknap of Fort Worth; his brother John Belknap (Cindy) of Fort Worth; and his niece Marisa Frear of Seattle, Washington..

In lieu of flowers, David's desire was for donations to be made to the Missions Fund at RockPointe.
