He is survived by: his wife Staci Shugart O'Quinn; his sons, Cody Ray O'Quinn, Jaxon Pirate O'Quinn and Chance Alan Forman (Julie); his granddaughters, Lillian Bijou and Charlotte Dior; his daughter Taylor Delain Grant (Brandon); his grandson Grayson Chance; his niece Norma Touchette (Jason); his great-nephew Myles; his nephew Michael Lee Wilson Jr. (Josie); and also Natalya.
