He was predeceased by: his parents, Frank Calvin and Lavonia Tappen (Akers); and his daughter Tammy Perkins. He is survived by: his wife Dawn; his sons, Theodore Samuel Tappen, William R. Tappen, Jr. and Frank Tappen; his siblings, Jackie Tappen and Eva Jo Dunn; and his former wife Judy Koehler (Chris). He is also survived by numerous grandchildren and great-grandchildren.
