He was predeceased by: his grandparents, Frank, Sr., Mary Polivka, John and Lorraine Millo; his uncles, Harlan Russell, Jr., James Ingrasin and John Millo; his aunt Rosemarie Russell; his cousin Steven Russell (Lisa); and his pets, Daisy, Lily and Dixie.
He is survived by: his parents, Patricia Polivka, and Frank Edward Polivka, Jr. (Ed) of La Marque, Texas; his son Frank Edward Polivka IV (Ivan); the mother of his child Desa Polivka; his sisters, Mary Lading (Mark) of La Marque and Nancy Simental (Carlos) of Tomball, Texas; his nieces and nephews, Scott Polivka (Angela), Matt Polivka (Lauren), Brandon Simental, Alex Lading, Jonathon Polivka, Jr., Brooke Lading, Courtney Simental, Tiffany Simental and Alyssa Polivka; his cousins, Tom Ingrasin (Jen), Catherine Ingrasin, Harlan (Trey), Jacob Russell, Lisa Delavecchio, Chris Millo, Nicholas Millo, Robyn Stawarz and Heather Fazzino; his aunts and uncles, Cynthia Ingrasin, Lula Millo, Edward Millo (Charlene), Frank Millo and Susan Fazzino (Charles); and his pet Harley.
