He is survived by: his wife Jodie; his children, Katelyn Kruciak and Dylan Kruciak; his granddaughter Evelynn; his great grandchild Todd Baker (Jill); his son Jason Baker; his brother Don (Sharron Bowman); his niece Nicci Fowler; his great-nephews, Gabe and Josh Fowler; his sisters, Karin Harris and Sandi Sykes (Albert Sykes); his niece Kandice Levental (Ilya); his great-nieces, Ailee and Emily; his niece Stephanie Cook (Dave); his great-nephew Andrew; his great-niece Alexandria; and also, Dwane Matthies, Allie and Kylie.

Instead of flowers, the family requests that memorial contributions be made in David's name to either Hope Lutheran Church, the American Cancer Society, or other cancer-fighting organizations.
