He is survived by: his wife Jessica Montalvo; his sons, Juan Miguel Montalvo, Jr. (Miguel), Jorge Gabriel Montalvo, Jayzel Marques Montalvo and Jovan James Montalvo; his mother San Juanita Sandoval; and his siblings, Jesus Montalvo, III (Josie Rodriguez), Jimmy Montalvo (Valerie), Jami Lee Rodriguez (Daniel), Joel Montalvo (Melissa Guzman), Jesse Joe Montalvo and Joshua Sandoval. He is also survived by numerous nieces, nephews.
