She is survived by: her husband Ronnie Culak; her children, Jason Culak (Tina), Chris Culak (Stephanie), Matt Culak and Eric Culak (Meagan); and her grandchildren, Dustin, Shane, Kirstin, Madelyn, Ashtyn, Lane, Mae, Alyssa, Dawson and Kameron.
