He was predeceased by: his father Charley Michalk, Jr.. He is survived by: his mother Paula Ann Hogan Michalk; his children, Rebekah Kelsey (Colby) and Reed Michalk; his siblings, Peggy Mosher (Kirk) and Kenneth Michalk; his grandchildren, Brooklynn Michalk, Clayton Kelsey and Claire Kelsey; and his nieces, Amanda Michalk and Sara Mosher. He is also survived by numerous great-nephews, great-nieces, cousins.
