He was predeceased by: his father Richard Edward Fleak. He is survived by: his mother Linda Jean Crawford-Bush; his wife Jessica Laureano-Fleak; his daughter Jaden Marie Fleak; his sister Kimberly Rae Fleak-Wouda (Peter); and his nephews, Justin Ray Johns and Brandon Robert Hurst. He is also survived by other loving relatives and friends.
