He is survived by: his parents, Deborah Sample and Ronnie Sample of Jasper, Texas; his wife LaQuisha Andrews of Jasper, Texas; his children, Forrest Andrews (Kristy) of Jasper, Timothy Gulley, Jr. of Jasper, Texas, Baleigh Andrews of Jasper and Jaleiah Cooper of Jasper, Texas; his siblings, Andy McClain (Kristin) of Granbury, Jay McClain of Cleveland, Texas, Allison McClain McGowan (Travis) of Harrisburg, Melissa Guy (Marshall Barton) of West Monroe, Misty Ray (Ryan) of Lumberton, Megan Kline (Matthew) of Houston and Nancy McClain-Collins (Dean) of Cleveland; and his grandchild Kambreigh Murphy.
