She was predeceased by: her parents, George Perkins and Susie Perkins; her husband Leslie H. Smith; her son Stephen W. Smith; and her siblings, Leo I. Perkins, Howard G. Perkins, Nelva Reed and Lillian Perkins.
She is survived by: her sister Gwendolyn Carlow; her children, Sarah Caye Smith and Howard Mark Smith; and her adoptive children, Steve Kiefer, Tammy Modgling and Christian McEntrye. She is also survived by numerous nieces, nephews and cousins.
In lieu of flowers, the family request donations be made in her memory to Peterson Hospice, Susan G. Komen Breast Cancer Foundation or the charity one's choice.
