He was predeceased by: his parents, Arthur John and Margaret Middleton; his parents-in-law, Dorothy and Elwyn Cooper; and his daughter Janet Middleton Kohavi.
He is survived by: his wife Jane Ann Cooper Middleton; his daughter Jamie Middleton of Washington; his son-in-law George of Alexandra; his granddaughters, Quinn Bailey of Washington and Shay Elise of Washington; his brothers, Clark Middleton (Madeleine), Paul Middleton and Steve Middleton; and his sisters-in-law, Sue Janway (Billy), Paz and Yvette Kohavi. He is also survived by sister.
The family has decided to postpone a formal memorial service for Mike until later in the summer due to concerns related to the pandemic.
