He was predeceased by: his parents, Clint Formby and Margaret Formby; and his sister Linda Kay. He is survived by: his sons, Jonathan (Kayla) of McKinney and Lane (Courtney Mitchell) of Houston; his wife Lisa Formby of Hereford; his grandchild Riley; his siblings, Brenda "Ben" Formby of New York City, Marshall (Betty) of San Antonio, TX and Scott (Kathy) of New York City; and his nieces, Alison Haley of Lubbock and Lauren Formby of San Antonio.
