She was predeceased by: her father Roy Samuel Culbertson; her father-in-law Paul Thomas Polizzo, Sr.; her stepfather James Donald Galloway; her granddaughter Addilynn Marie Polizzo; her grandparents, George Hilton Ford and Sadie Mae Ford; her uncle Keith Ford; and her cousin Justin Ford.
She is survived by: her mother Gerry Terese Culbertson Galloway of Decatur, Texas; her husband Pat Polizzo of Haslet, Texas; her children, Kendra "Droopy" Polizzo of Lindsay, Texas, Sam Polizzo of Haslet, Texas, Andrew Polizzo of Haslet, Texas and Hannah "Girly" Polizzo of Haslet, Texas; her sisters, Suzy Matthews of Decatur, Texas and Rachel Markham of Haslet, Texas; her granddaughter Bristol of Keller, Texas; and her mother-in-law Elaine Polizzo of Haslet, Texas. She is also survived by 2 brother-in-laws, and many nieces and nephews.
