He was predeceased by: his mother Gerry Christine Polizzo; his niece Addilynn Marie Polizzo; his grandfathers, Paul Thomas Polizzo, James Donald Galloway and Roy Samuel Culbertson; and his great-grandparents, Sadie Mae Ford and George Hilton Ford.
He is survived by: his girlfriend Jessica "Jesse" Marie Green; his father Pat Polizzo of Haslet, Texas; his siblings, Kendra Polizzo of Lindsay, Texas, Sammy Polizzo of Haslet, Texas and Hannah Polizzo of Haslet, Texas; his grandmothers, Elaine Polizzo of Haslet, Texas and Gerry Galloway of Decatur, Texas; his niece Bristol Polizzo of Keller, Texas; and his aunts and uncles, Paul, Georgie Polizzo of Haslet, Texas, Jeff, Wendy of Keller, Texas, Rachel of Haslet, Texas and Skyler of Decatur, Texas. He is also survived by many loving family members and friends.
