He was predeceased by: his parents, William Polk and Bonnie Polk (Black); and his brother Glen Polk. He is survived by: his children, Mike Lee Polk of Tokio and Marissa Polk of Waco; and his nieces, Shelly and Melinda. He is also survived by many other relatives and friends.
