He was predeceased by: his parents, Julius R. Mikulenka and Mary Sitka Mikulenka; his wife Maxine; his siblings, George Mikulenka, Julius Mikulenka, Loraine Marie Butschek and Anna Louise Butschek; and his granddaughter Nicole Mikulenka.

He is survived by: his sons, Donnie Mikulenka (Linda) and Randy Mikulenka; his sister Mary Lee Brosch; his friend Elrose Kalisek; and his daughters, Melissa Holub, Gayle Kubenka and Patricia Klesel. He is also survived by 4 grandchildren; and 2 great grandchildren.
