He was predeceased by: his wife Rose Self. He is survived by: his children, Andy Wayne Self (Nichole Ann) and Rebecca Sue Sayer (John); and his stepchildren, Dodie Peek (Jerry), Hal Brown (Ronda) and Tommy Brown (Michelle).

Interment: Graford Cemetery, In Lieu of flowers, the family requests donations be made to Charlie's Angels.
