He was predeceased by: his parents, Clarence Edward Fail and Lucille Boyd Fail; and his sister Geraldine Fail Causey. He is survived by: his wife Mary Sue Fail; his children, Anthony, Juanita Fail, Samantha, David Mayberry and Aaron Fail; his brothers, Gary and Ricky; and his grandchildren, Isaac Fail, Rachel (Bryan Grotheim), Madison Fail and Naeya Mayberry.
