He was predeceased by: his parents, Richard Forbes Forsythe (Scotty) and Esther Lucille Ferguson; and his siblings, Charles Forsythe, Douglas Forsythe, Barbara Zots and Ralph Ferguson.

He is survived by: his wife Bobbie Sue Forsythe of Georgetown, Texas; his children, Lisa Gayle Forsythe (Rick Williamson) of Georgetown and Donald Forbes Forsythe, Jr. (Jenny Forsythe) of New Braunfels, Texas; his granddaughters, Sarah Gayle Forsythe and Annebelle Grace Forsythe; and his extended family, Ellen Forsythe, Stephanie Grenon, Jessica Smith and Fletcher Lowe.
