He was predeceased by: his wife Pauline Roberta Cusack; and his brother John Patrick Cusack, Jr.. He is survived by: his sons, Michael Francis Cusack, Jr. (Donna), Christopher Cusack (Marla Cusack) and Patrick Cusack (Lisa); his grandchildren, Brendan Cusack, Cameron Cusack, Michael Cusack, III, Ethan Cusack, Connor Cusack and Clara Cusack; and his close friend Happy Shahan of Brackettville.
