He was predeceased by: his parents, Dwight Earl Leavens and Phyllis Irene Chambers Leavens; and his brother-in-law John Glenn Mayer. He is survived by: his siblings, Gary D. Leavens (Sandra) of Jasper, GA and Karen S. Leavens Mayer of Maylene, AL; and his nieces, Caitlyn E. Mayer Seagraves, Alyssa B. Mayer Onkka (Ryan), Lauren G. Mayer Dunn (Madison), Emma R. Mayer Lucas (Carlton) and Tess E. Mayer.
In lieu of flowers, the family has asked that donations be made to the American Cancer Society in his memory.
