He is survived by: his wife Shelia Ann Cook; his daughters, Angela Dee Cook and Jennifer Lynn Perry; his son-in-law Chris; his grandchildren, Abigail Lynn, Ethan Michael, Andrew David, Elisabeth Ann Seifried, Colin David, Victoria Ann and Madalynne Gail Perry; and his siblings, Fulton Cook (Rose) of Saginaw, Dennis Cook of Alvarado, Martha Pitman (Jerry) of Rochester, Lydia Weido (Bob) of Smackover, Daniel Cook (Connie) of Bedford and Deborah Gaddy (Walter) of Forest Hills. He is also survived by numerous nieces, nephews and friends.
