He was predeceased by: his grandparents, Dick, Shirley Burke, James Holt, Ronnie Witt, Sonja and Gene Burkhart.

He is survived by: his daughter Blayke Avery; his wife Kelly Cummings; his parents, Rick Burke of Follett and Shelley Burke of Follett; his sister Brandi; his brother-in-law Casey Cowan; his nephews, Cohl and Caine of Balko, Oklahoma.; his sister Mindi; his brother-in-law Lynn; his nieces and nephews, Burkley and Greyson Scott of Dumas, Texas.; his uncles, Craig (Susan Holt) and Jeff (Linda Holt); his father-in-law Wally (Holly Cummings); his sister-in-law Katy (Zack); his nephew Jackson; his niece Stella Rogers of Owasso, Oklahoma.; and his brother-in-law Christopher Cummings of Tulsa.
