He was predeceased by: his father Elmer; his wife Jean; his sister Elsye Addy; his sons-in-law, Larry Moorman and Rod Castleberry; and his grandson Richard Castleberry.

He is survived by: his children, Milton (Avis) of Nocona, Texas, Judy Moorman of Seguin, Texas, Donna Castleberry of Luling, Texas and Jeanice McCommas (Rick) of Lockhart, Texas; his sister Lera Wilson of Smyer, Texas; and his grandchildren, Lex Frazier, Wako Frazier, Kasey Frazier, Tishell Fontenot, Kole Frazier, Cody Moorman, Shonda Briere, Beni Muffler, Chris Castleberry, Nicole Hage and Garrett McCommas.

In lieu of flowers, the family asks that you donate to Jack's favorites charity, St. Jude Children's Hospital.
