She was predeceased by: her parents, Charles Dziuk and Agnes Dziuk (Sekula); and her siblings, Leonard Dziuk, Joe Dziuk, Sylvia Scheffler, Virgie Lyssy and Edwin Dziuk.
She is survived by: her husband Anton "Tony" Urbanczyk; her children, Connie Moy (Kenneth), Brian Urbanczyk (Terrie), James Urbanczyk (Elizabeth) and Bradley Urbanczyk (Erika); her grandchildren, Morgan Urbanczyk, Reagan Urbanczyk, Aaron Moy, Berklie Urbanczyk, Reese Urbanczyk, Steel Urbanczyk and Jackson Urbanczyk; her siblings, Clarence Dziuk (Gladys), Charles Dziuk (Dolores) and Joan Witte (Leon); and her siblings-in-law, Theresa Dziuk and John Lyssy.
In lieu of flowers, the family asks that memorial donations be made out to St. Ann's Catholic Church.
