He was predeceased by: his parents, Anton Slovak and Justina "Tinnie" Slovak (Halencak); his wife Ann Thomas; and his siblings, Frank Vanek, Johnny Slovak, Jerry Slovak, Jimmy Slovak, Shirley Johnson, Dorothy Slovak and Patricia Slovak.
He is survived by: his pets, Pumpkin and Tiger; his children, Russell Slovak (Lojuana) of Grand Prairie, Leann Mordecai (Micah) of McGregor and Beverly Pilgrim of Elm Mott; his siblings, Bob Slovak (Judy), David Slovak (Janell), Daniel Slovak (Theresa), Ruth Gerik and Gladys Jares (Melvin); his grandchildren, Destinee Mordecai, Taylor Roland (A. J.), Tanner Mordecai, Corey Slovak and Leanna Pilgrim; and his great grandchild Micah Liam Terry. He is also survived by many nieces, nephews, other relatives and friends.
In lieu of flowers, memorials may be made to St. Mary's School Endowment Fund.
