He was predeceased by: his parents, Charles Hurst Sample, Sr. and Frances Billups Sample; and his siblings, Edna Frances Sample and Charles Hurst Sample, Jr.. He is survived by: his wife Maureen Davis Sample; his children, Doug Sample (Karri), David Sample (Marla) and Michelle Sample; his grandchildren, Tyler Sample, Zachary Sample, Landry Sample, Addie Sample, John Sample and Jackson Sample; his nephews, Steve Sample, Kent Wade and Nathan Edwards; his cousin Desmond Powell; his nephew Jon Charles Sample; his friends, Ron Taylor, David Rose and Charles Placker; and also Samantha Reyes.
