He was predeceased by: his parents, Ray Lewis and Anna Marie Lewis. He is survived by: his wife Martha Jo Lewis; his daughters, Erin Lewis Freeman and Leslie Terry; his daughter-in-law Molly Freeman; his brother Dennis Ray Lewis (Judy); his brother-in-law William Andrew Marr; and his grandchildren, Patrick L. Terry, Sean Hollis and Sydnee Terry. He is also survived by numerous relatives and friends.
