He was predeceased by: his parents, Willie Ray and Jannie Juanita Headrick; and his brother Bobby Johnson. He is survived by: his wife Patricia Headrick; his children, Jessie Slaughter, James Dale Headrick (Judy), Janelle Shipman and John Headrick; and his siblings, David Johnson and Glenda McDonald (Jerry). He is also survived by numerous nieces, nephews, other relatives and friends.
