He is survived by: his children, Megan Cook of Orlando, Florida and Kevin Cook of Playa Grande; and his siblings, Sharon Hazelton of Dallas, Texas, Esther Neely (Kent) of Dallas, Oregon and Ed Cook (Billie) of Yukon, Oklahoma. He is also survived by numerous nieces, nephews, and many friends.
